I authorize Alpha Roadside Service to perform the repair work described by an Alpha Roadside Service employee in a written or verbalized estimate utilizing necessary labor, parts, and materials. I agree that Alpha Roadside Service is not responsible for loss or damage to the vehicle, or articles left in the vehicle, in case of theft, fire, or any other cause beyond their control.

I agree that Alpha Roadside Service is not responsible for delays caused by the unavailability of parts, or delays in parts shipments by the supplier or transporter. I grant Alpha Roadside Service employees or vendors permission to operate my vehicle for the purpose of testing and/or inspection. I understand that, if any closer analysis finds additional labor, parts or materials are necessary to complete the repair, I will be contacted for authorization.

I acknowledge that this work authorization approved supplemental estimates. I agree that if I should halt repairs for any reason, I will be responsible for the cost of any and all repairs completed to that point, as well as the cost of the parts which are not returnable, or restocking fees charged to Alpha Roadside Service, if I chose not to purchase said parts outright. I am entitled to retain any parts I pay for that are not returnable to their vendors.

*Normal Callout Fee: $119.00 (6am-8pm)
*After Hours Callout Fee: $129.00 (8pm-6am)
*Drive To & From Fee: Is charged at the hourly Rate (Subject to change due to distance/weather)
*Normal Hourly Rate (for the first 2hrs): $120.00/hour (6am-8pm)
*Normal Hourly Rate (After the 2hrs): $150.00/hour (6am-8pm)
*After Hours Rate (for the first 2hrs): $150.00/hour (8pm-6am)

I authorize Alpha Roadside Service to charge the credit card according to the terms outlined above. This payment authorization is for the goods/services described above, for the amount indicated in the Work & Credit Card Authorization, and is valid for one-time use only. I certify that I am an authorized user of this credit card and that I will not dispute the payment with my credit card company; so long as the transaction corresponds to the terms indicated in this form.

Terms of Service:

By accepting the Terms Of Service I understand that not all vehicles can be repaired via roadside service and may need to be taken to a shop for further diagnosis. Also, note that there will be no guarantees or warranties given on any roadside service. There will be absolutely no refunds on services completed. Alpha Roadside Service might use third-party vendors to perform our mechanical services. No payment is to be made to the vendor directly during this agreement. If payment is made to the vendor directly, Alpha Roadside will charge a 50% management fee for managing your breakdown. You may cancel this authorization within 10 minutes from the time of the request at no additional cost by calling 855-457-1170. Once this form is submitted and the call is canceled, a $250 cancellation fee plus the credit processing fee will apply. Please note that the cancellation fee is non-refundable.
